To help make the process as stress-free and straightforward as possible, here are a few things you need to know about your first visit with us.
Forms We Will Need
The New Patient forms you fill out contain a lot of valuable information and start your patient record with us. You can streamline your first visit with us by downloading and printing your forms at home and bringing them with you to your first visit.
You can find our both our New Patient forms and Health History Update form on our Forms page. If you cannot complete your paperwork beforehand, please plan to arrive 15 minutes before your appointment so you can get the most out of your time with the dentist.
Insurance Information
Please bring a picture ID and any insurance cards you plan to use.
Checking In
When you arrive at our comfortable, homey office, you will be greeted warmly by a member of our front desk staff who will check you in, give you the forms you need to complete, and get you settled in our comfortable reception area.
Your First Appointment
When it’s time for your appointment, you will meet with Dr. Misato or Dr. Venuti. A critical part of your first visit is the time we take getting to know you to understand what’s important to you and how we can help you. We never rush this process and will be happy to answer all your questions thoroughly.
Every first appointment includes an exam and dental cleaning so our doctors can evaluate the condition of your teeth and gums. If we find problems that we need to address, we will work with you to establish a treatment plan to get your oral health back on track.
Before you leave, we will talk to you about how your dental insurance applies, explain financing options like CareCredit and arrange subsequent appointments.
